Greetings All,

I am writing today to bring you all up to speed on the ever-changing directives issued by the PA Department of Health as they pertain to dentistry in our state.

On Friday, May 15, in a clarification of a vaguely worded document released on May 8, 2020, the DOH stated that “cleanings for low risk patients should not be performed at this time. However, cleanings may be done in situations in which there is clear clinical need, as part of a larger medical treatment plan. For example, if a patient needs to have a cleaning to be medically cleared prior to planned surgery, or if a patient is periodontally compromised (to prevent irreversible damage).”  Unfortunately, that leaves many of you unable to have preventive treatment with a dental hygienist which is the standard of care we have all come to expect in the age of modern dentistry. However, some of you should still continue your preventive therapy at the risk of worsening periodontal disease (ex. bone loss, gum recession, excessive tarter build-up with associated inflammation).

Cumulatively, my staff and I have completed hours and hours of online dental webinars during the past six weeks presented by some of the foremost experts in infectious disease in the world. We decided that we would provide hygiene care to periodontally compromised patients without generating any aerosols, which is a concern with airborne infections, by limiting our treatment to the scaling of teeth with hand instruments in the presence of intraoral high-speed evacuation systems where needed.  We learned along the way how to protect ourselves and our patients if, in fact, someone with the virus enters our office. We had multiple ZOOM meetings together as well as in-office meetings to initiate our new protocols to address the possibility of airborne pathogens.  We are diligently screening ourselves and our patients daily. We’ve installed air purifiers with HEPA filtration and UV light in every treatment room. We are fogging all operatories with hypochlorous acid immediately after patient visits.   We are wearing PPE as prescribed by experts.  And we are fastidious with hand and personal gear hygiene. We have done as much as feasibly possible to ensure the safety of our patients and staff.  And, yet, the DOH persists in its dictatorial directives and treats every county in the state as if it’s the epicenter of the pandemic.

Though I fully understand the need for caution, I wanted to reassure you once again that when more practical guidance resurfaces, we are well-prepared to provide safe preventive dental care at this chaotic time of our lives.

Through my first 40 years of life, going to the dentist was one of my least favorite and most fearful of activities … until I came upon Dr. Fishbaine.  My first visit to his office was over 30 years ago. I was overwhelmed by his patience, gentility and overall general concern for my comfort to the extent that, not only did I start to see him on a regular basis, but, as soon as my kids turned 2 and 3 years old, I had no qualms about taking them for Check-ups and, not surprisingly, they, as well of my Dear Wife, Dianna, just loved him!
